How to make the insurance claim
Step 2 (Fill out the insurance claim form)
- Download the relevant form and provide all required documents for your claim
- Please fill out all required forms and require the attended physician to complete a medical report

- Please answer all the questions in the Forms and attach all required documents.
- Certification of documents can be done by sighting the original documents by the authorized personal (Insurance advisor or Manulife Agency Manager)
- Depending on the nature of claim, we may ask you for additional information if needed.
