How to make a claim

  • Your claim is our priority, we strive to ensure that all valid claims are paid promptly.
  • We encourage our customer to submit all necessary claim documents to us soonest possible to help speed up the claim process.

Step 1

Notify us for your loss event

  1. Call: 1800 211 211 (Toll Free Number). Our claims hot-line staff are ready to serve you if there is a phone call asking for “How to submit claim”.
  2. Email: claims_service_kh@manulife.com
  3. Walk-in to the nearest Manulife Office

Check the required documents for your claim and download the relevant form(s)

  1. Death Claim
  2. Total Disability Claim
  3. Critical Illness Claim
  4.  Manulife Medicash 

Important Notes

  • If claim occurs within 2 years from the policy inception or reinstatement date, please provide us with name/ address/ contact number of the insured’s regular attending doctor for our further due diligence. We will appreciate if a copy of documents treating is provided.
  • Please complete the claim form by answering all questions and attach relevant documents required above.
  • All expenses insured in getting claim documents are to be borne by the Claimant.
  • Certification of documents can be done by sighting the original documents by the authorized personal (Manulife’s Personal or Agency Manager)
  • Depending on the nature of the claims we may ask for additional information if necessary.  

Step 2

Complete the required form(s) accurately & get your treating doctor to complete the medical report (if any) 

Step 3

3 options to submit the completed form (s) and supporting documents:

  • Mail to Claim Department Manulife Insurance 1st floor, Siri Tower 104, Russia Federation Boulevard, Sangkat Toeuk Laak I, Khan Toul kork, Phnom Penh, Cambodia
  • Submit at your nearest Manulife office  
  • Submit through your servicing agent
  • Through emails: claims_service_kh@manulife.com

Download forms:

  1. Claim form & Authorization
  2. Death Claim - Attended Physician’s Statement
  • To be completed by the last attended doctor/physician for a death that took place.
  • Required for any death claim happened within 2 year of policy issue date or re-instated date.    

Documents required:

  • Original or certified true copy of Death Certificate

Note: Issued by the local authorities (Commune/Sangkat) or Cambodian embassy abroad (if the case is overseas)

  • Original contract provision or declaration of lost policy contract (Not applicable for e-Contract)
  • Evidence of medical treatment and diagnosis documents

Note: All medical treatment documents such as Doctor certificate, histopathology report, and all laboratory results.

  • Proof of identity documents of Life Insured and Beneficiary(ies) or the person claiming the benefits

(Copy of ID/Passport with fresh-ink signature)

  • Proof of the claimant’s relationship to the deceased (Proof of relationship to the deceased such as family book, residential book, birth/wedding certificate…….) Note: Will not require if it is provided previously
  • Unnatural death or for death occurred overseas
  1. Competent police/authorities report (if the death caused by accident)
  2. Newspaper clipping (if any)
  3. Autopsy & coroner report (if the death resulted by suicide or homicide or any specific unnatural cause of death)
  4. Certified true copy of burial / cremation documents (if the death occurred overseas)

  2.  Total Disability Claim - Attending Physician’s Statement

Note: To be completed by the attending doctor to confirm the disability of the Life Insured in permanent. 

Documents required:

  •  Proof of Total Disability

Note: Medical Certificate or any equivalent medical documents to confirm that Life Insured meets the definition of total disability and it is issued by licensed doctor or hospital.

  • Evidence of medical treatment and diagnosis documents

Note: All medical treatment documents such as Doctor certificate, histopathology report, and all laboratory results.

  • Proof of identity documents of Life Insured and Beneficiary(ies) or the person claiming the benefits

(Copy of ID/Passport with fresh-ink signature) Note: Will not require if it is provided previously

  • If the claim event caused by accident 
  1. Competent police/authorities report (if the death caused by accident) 
  2. Newspaper clipping (if any)

  • Claim form & Authorization (Note: Not applicable for EasyClaim)
  • Hospital Income - Attended Physician’s Statement (if it is necessary for claim decision)

Note: To be completed by the last attending doctor

Documents required:

Mandatory requirements: 

  • Medical Discharge Summary/Letter
  1.  Issued by Hospital to confirm that the Life Insured was hospitalized for a period and indicated the Life Insured’s Diagnosis.
  2. This document requires hospital/clinic official stamp and signature of the Attending Physician
  • Hospital Receipt/Invoice
  1. Hospital billing which is charged the patient for all service including room and board, ICU service…etc.

Additional requirements:

  • Evidence of medical treatment and diagnosis documents

Note: All medical treatment documents such as Doctor certificate, histopathology report, and all laboratory results.
