Individual insurance claim

  • Your claim is our priority, we strive to ensure that all valid claims are paid promptly.
  • We encourage our customers to submit all necessary claim documents to us soonest possible to help speed up the claim process. 

How to make the insurance claim

Please submit the completed forms and requirement through: 

  • Claim Department: 13th Floor, TK Central, No. 12, Street 289, Sangkat Boeung Kak 1, Khan Toul Kork, Phnom Penh. Kingdom of Cambodia
  • The nearest Manulife office
  • Submit through your servicing agent

 

Verify the documents needed to apply for your claim and download the relevant form

Important notes 
  • Please answer all the questions in the Forms and attach all required documents.
  • Certification of documents can be done by sighting the original documents by the authorized personal (Insurance advisor or Manulife Agency Manager)
  • Depending on the nature of claim, we may ask you for additional information if needed.
